The Cigna Group lifted its 2026 floor for adjusted income from operations per share to at least $30.60 after third-quarter revenue rose 6%, as health and specialty gains helped offset weaker pharmacy results.
The increase was 15 cents from the previous minimum. Revenue for the three months ended Sept. 30 reached $73.683 billion, compared with $69.748 billion in the same period of 2025. The company separately reported adjusted revenue of $73.793 billion.
Adjusted income from operations edged up to $2.104 billion, or $7.98 per share, from $2.096 billion, or $7.83. Shareholders’ net income fell to $1.442 billion from $1.868 billion. Cigna said the year-earlier result included an after-tax gain on a sale that did not recur.
Evernorth Health Services’ adjusted revenue rose 6% to $63.774 billion. Specialty and Care Services’ pre-tax adjusted income from operations increased 15% to $1.064 billion, while Pharmacy Benefit Services’ measure fell 22% to $765 million from $975 million. Cigna said major client renewals and strategic investments weighed on the pharmacy benefits result.
Cigna Healthcare’s adjusted revenue grew 9% to $11.708 billion, and its pre-tax adjusted income from operations rose 15% to $1.193 billion. Cigna linked the revenue increase mainly to premium adjustments designed around anticipated medical-cost growth. It also said wider use of biosimilars and specialty generics lowered costs for clients and patients, supporting Specialty and Care Services.
For 2026, the company set minimum pre-tax adjusted income from operations of $6.9 billion for Evernorth and $4.6 billion for Cigna Healthcare. The Cigna Healthcare minimum increased by $50 million, while Evernorth’s remained unchanged. Cigna projected an 84.2% medical care ratio for Cigna Healthcare, with a range of 25 basis points above or below that level. The ratio compares medical costs with premiums for specified risk products.
Reuters, citing LSEG estimates, reported that Cigna’s adjusted revenue of $73.79 billion exceeded a $72.72 billion estimate. Adjusted earnings of $7.98 per share also topped the $7.50 estimate.
Cigna plans to exit its Individual and Family Plans medical business effective Jan. 1, 2027.
