MOORESTOWN, N.J., Aug. 29, 2017 -- Tabula Rasa HealthCare Corporation (NASDAQ:TRHC), a healthcare technology company disrupting the field of medication safety, announces that the largest Program of All-Inclusive Care for the Elderly organization (PACE) has signed an agreement for medication risk management services with CareKinesis, its wholly owned subsidiary focused on PACE. CenterLight Healthcare PACE selected TRHC to work with the Interdisciplinary Care Team (IDT) to streamline workflows and improve outcomes for participants. The agreement will begin with a launch center with additional locations to be added over time in consideration of specific clinical, economic, and satisfaction-related outcomes.
CenterLight Healthcare’s PACE Organization is a vanguard of the community physician model that enables patients to keep their primary care physician once they enroll in PACE. This model is now enabled through a waiver program. CMS is considering eliminating the waiver in its newly proposed conditions of participation to increase access to PACE. TRHC will collaborate with CenterLight network and employed prescribers and staff to reduce hospital admissions, re-admissions and total costs while increasing participant and provider satisfaction.
