Washington, D.C., Sept. 28, 2017 -- The Cancer Support Community (CSC) and the LIVESTRONG Foundation (LIVESTRONG) today announced the launch of the next generation of their highly-regarded resource, Cancer Transitions: Moving Beyond Treatment, a free education program to help cancer survivors make the successful transition from active treatment to post-treatment and the next chapter in their lives.

Over the past decade, Cancer Transitions reached thousands of patients in over 150 locations globally, through more than 425 completed programs. The updated Cancer Transitions incorporates the latest research on maintaining mental and emotional health, eating well, and exercising. During the program, facilitators will also educate participants on Survivorship Care Plans, records of a patient's cancer history and recommendations for follow-up care and lifestyle changes. The Commission on Cancer now requires cancer centers to provide Survivorship Care Plans to all eligible patients.
Available beginning September 28, the new Cancer Transitions is made possible through generous support from Cancer Treatment Centers of America® (CTCA), a national network of five hospitals that serves adult patients who are fighting cancer. Additional funding was provided by Genentech.

Cancer Transitions, which is designed for survivors of any cancer diagnosis who have completed treatment within 24 months (although survivors who are further past treatment may participate), includes six weekly 2.5-hour sessions covering such topics as: exercise, nutrition, relaxation, stress management, emotional and social support, and long-term medical management.
