Medical practices leverage the NCQA prevalidation process to identify health IT solutions that will help them meet NCQA PCMH Program requirements. Practices that obtain PCMH recognition become eligible for additional incentives from The Centers for Medicare & Medicaid Services (CMS).

NCQA PCMH Prevalidation included a rigorous evaluation of CipherHealth’s automated outreach solution, care management platform, and point-of-care patient survey application.
The solutions are PCMH prevalidated for a total of nine criteria in four PCMH concept areas: Patient-centered Access and Continuity, Knowing and Managing your Patients, Care Management and Support, and Care Coordination and Care Transitions.
