Primaria Health provider partners are now engaged with the Centers for Medicare and Medicaid Services (CMS) to continue to improve quality and reduce costs for its approximately 30,000 aligned Medicare beneficiaries. Primaria Health will continue to provide its physician partners with the solutions and services they need to improve outcomes, enhance patient and physician satisfaction, and lower the total cost of care.

Primaria Health initially formed a Medicare Shared Savings Program Accountable Care Organization (ACO) in 2017 and is the largest group of primary care physicians in Central Indiana with 400 provider partners. Moving to a Medicare Next Generation ACO Model in 2018, the group can expand its care coordination tools and resources to address the needs specific to seniors as well as support them in achieving performance metrics for value-based contracts.

Primaria Health’s provider partners will be building upon their demonstrated clinical improvements in the areas of diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and other complicated health conditions. Primaria Health brings sophisticated data analysis to identify beneficiaries at most risk for a variety of health conditions and complications, as well as those living with chronic conditions.
