ALBANY, N.Y., Aug. 1, 2016 -- New York's Capital Region is once again making headlines for health care innovation. The Centers for Medicare and Medicaid Services (CMS) today announced that New York's North Hudson/Capital Region is one-of-14 markets across the country selected to participate in a new nationwide primary care model.
Comprehensive Primary Care Plus (CPC+) is a five-year primary care medical home model that pays primary care physicians based on the quality of care they provide their patients. According to CMS, CPC+ is the "largest-ever initiative to improve primary care medicine in America."
CPC+ is a multi-payer model which includes Medicare, Medicaid, and private insurance companies. CMS selected the regions based on payer interest and coverage.
"CDPHP is proud to once again partner with CMS on this important initiative. Our health plan helped lay the foundation for agency's selection of our region with the launch and development of our Enhanced Primary Care (EPC) program, a patient-centered medical home model that rewards doctors for spending more time with their sickest patients. Now in its eighth year, EPC includes over 200 primary care practices, nearly 900 clinicians, and more than 230,000 CDPHP members. A recent study found EPC saved $20.7 million in medical costs, all while improving the quality of care for hundreds of thousands of local patients," said CDPHP president and CEO, Dr. John D. Bennett.

3. Hawaii: Statewide
4. Kansas and Missouri: Greater Kansas City Region
5. Michigan: Statewide
6. Montana: Statewide
7. New Jersey: Statewide
8. New York: North Hudson-Capital Region
9. Ohio: Statewide and Northern Kentucky Region
10. Oklahoma: Statewide
11. Oregon: Statewide
12. Pennsylvania: Greater Philadelphia Region
13. Rhode Island: Statewide
14. Tennessee: Statewide
Building on the Comprehensive Primary Care initiative that launched in late 2012, CPC+ will benefit patients by helping primary care practices:
• Support patients with serious or chronic diseases achieve their health goals
• Give patients 24-hour access to care and health information
• Deliver preventive care
• Engage patients and their families in their own care
• Work together with hospitals and other clinicians, including specialists, to provide better-coordinated care
