LAFAYETTE, La., Feb. 10, 2016 -- LHC Group (NASDAQ:LHCG) and Baton Rouge General today announced a new joint venture agreement that will enhance home care service in Baton Rouge and help to seamlessly transition patients from the hospital to the home.

Last November, Baton Rouge General released its “Future of Healthcare” report, which outlined its plans to collaborate with other providers to improve community health and the convenience and affordability of care. Hospital and home health partnerships are one key component of helping patients manage their conditions. More than 60 leading hospitals and health systems around the country have partnered with LHC Group to deliver patient-centered care in the home.
Patients with certain conditions are referred to home health as a part of recovery. Physicians then work closely with the home care team to deliver quality, compassionate care designed to help patients transition seamlessly into daily life. In addition to offering health care services, home health professionals also educate patients on their conditions, medication management and nutrition.
