Create’s products are offered to commercial, labor, and public sector plan sponsors as an alternative to traditional health plans, allowing families to shop on a user-centric, cloud-based platform for a health care system as their exclusive provider, such as Hackensack Meridian Health, resulting in higher-quality care, immediate lower costs, and stronger relationships between patients and doctors. Through Create’s innovative model, the interests of employers, patients, families, physicians, and hospitals are aligned, helping to improve quality and coordinate care, while reducing unnecessary costs.

Data exchanges between Create and Hackensack Meridian Health and its clinically integrated network enable timely and proactive outreach by Hackensack Meridian Health to families that choose to enroll in their system.
